Clinic patient records + AI notes

The challenge

Therapists juggled paper notes, shared drives, and unstructured Word docs. Finding prior session context took too long, and documentation quality depended entirely on who was writing.

What we did

1. Designed Cogni Arkiv around clinician workflow first: intake, session notes, history, and follow-ups.
2. Hardened access with account roles, audit-friendly history, and clear separation between patient records and admin tools.
3. Added AI-assisted note summaries clinicians can review and edit—never auto-publish without a human check.
4. Trained the clinic team and left playbooks for onboarding new therapists without IT tickets.

The outcome

Clinicians reclaim hours each week, patient history is searchable in one place, and documentation is consistent enough for better continuity of care.
